Use this form to:

  • Request new account creation for a new employee hire, contractor, consultant, or student

  • Request account deactivation for a departing employee, contractor, consultant, or student


Upon filling out the respective fields a ticket will automatically be created and submitted to ITS with the following information: 



For onboarding a new hire:


  • Requestor name
  • Employee affiliation (full time, third party)
  • Employee name
  • Employee title
  • Department/center 
  • Manager
  • Start date
  • Employee location
  • Room/Office/Cube number
  • Penn phone activation
  • Computer needs
  • listserv subscriptions needed
  • Shared drive access needed
  • Application access



For offboarding a departing employee:

  • Requestor name
  • Employee affiliation (full time, third party)
  • Employee name
  • Employee email
  • PennKey username
  • Department/center 
  • End date
  • Would you like an out of office set for the departing employee in O365 Outlook, voicemail or both?
  • Does the departing employee have a work issued mobile phone?
  • Computers/devices to return
  • Return date for computers/devices
  • Services to disable
    Please list any storage services such as Penn Box, conferencing software such as Zoom or Bluejeans or any social media platforms to be disabled.


Notes:

  • PennKey usernames: New full time university staff will be sent an email from the Workday system several days before their start date. In that email, the new hire will be given instructions to create their new PennKey.
  • ITS is unable to create most new network, email, and other accounts until the new hire PennKey name is established.
